Getting Started

 

You are able to see the physical therapist without a prescription from your doctor or specialist but it is unlikely that your insurance will pay for these services without a referral.  Please contact your doctor and ask for a prescription before scheduling your first appointment.  If you need assistance in obtaining a referral, please contact us, we would be glad to help.  If you prefer to pay for services privately, please contact our office to make arrangements.

You may download a printable prescription to take to your next doctor's visit. 

Forms:

Once you have received a referral for physical therapy, please contact our office (801.479.5777) to schedule an appointment.  If possible, please download, print, and complete the following forms and bring them to your appointment:

• Patient Information

• Privacy Notice

• Authorization of Benefits

• Cancellation/No Show Policy

• Dizziness Scale (only for patients being treated for dizziness/balance disorders)

*Please bring your insurance card to your appointment.*

How long will my appointment take?

Your first appointment will take 60 to 90 minutes.  Please arrive a few minutes early to complete any paperwork.  Any additional appointments will take approximately 1 hour.

What should I wear?

Wear comfortable clothing and comfortable shoes to move around in.
